depends on the builder you can get 700r4 stronger than a 2004r and vice versa
Going by finger tight only isn't good enough, you need to make sure it is going in the right depth or you can tear the threads out

I also agree about the rubber end gaskets being bad. Make sure the silicone squidges into the corners good.
If it only broke after the bolt got loose I would say the bolt could be causing the problem!
